Most people get an annual physical, have blood drawn, and a few days later hear some version of "everything looks normal." That's reassuring — but it can also be misleading. You can walk away with a clean bill of health on paper and still feel tired, foggy, or stuck with stubborn weight gain you can't explain.

The issue usually isn't that anything was done wrong. It's that a standard panel — a complete blood count, basic metabolic panel, lipid panel, and maybe a TSH — was designed primarily to catch disease: anemia, kidney or liver dysfunction, diabetes, high cholesterol. Those are important. But they were never designed to tell you whether your metabolism, hormones, or inflammation levels are actually working in your favor.

What a standard physical typically checks

A typical annual panel covers red and white blood cell counts, basic kidney and liver markers, electrolytes, fasting glucose, total cholesterol with LDL/HDL/triglycerides, and often just one thyroid number — TSH. Each of these has real value for catching overt disease. But on their own, they leave significant blind spots for anyone trying to understand why they feel the way they do, or trying to optimize their health rather than just rule out illness.

Six biomarkers that often get missed

1. Fasting insulin & HOMA-IR
Glucose can stay "normal" for years while insulin quietly climbs. Fasting insulin and HOMA-IR can flag insulin resistance long before fasting glucose or A1c become abnormal.
2. hsCRP (inflammation)
High-sensitivity C-reactive protein is a marker of low-grade chronic inflammation — linked to cardiovascular risk, metabolic dysfunction, and how you feel day to day. It's rarely included in a standard panel.
3. Full thyroid panel
TSH alone can miss real thyroid dysfunction. Free T3, Free T4, reverse T3, and thyroid antibodies (TPO, TgAb) give a much more complete picture of how your thyroid is actually functioning.
4. Sex hormones
Testosterone, estradiol, SHBG, and DHEA-S all influence energy, mood, sleep, and body composition — for men and women. These are almost never checked in a routine physical.
5. Advanced lipid markers
ApoB and Lp(a) are increasingly recognized as better predictors of cardiovascular risk than standard LDL alone — especially for people with a family history of heart disease.
6. Micronutrient status
Vitamin D, B12, ferritin, and magnesium deficiencies are common, often asymptomatic until they're not, and are a frequent — and correctable — cause of fatigue and brain fog.

The "normal" range problem

Lab reference ranges are built from a statistical sample of the general population — which includes a lot of people who are overweight, sedentary, or have undiagnosed metabolic issues. "Normal" reflects what's common, not necessarily what's optimal. A result that falls within the reference range can still be far from where it would ideally sit for someone aiming to feel and function their best, not just avoid a diagnosis.
